If your child has multiple chronic conditions or special health care needs, there is a special program for Keystone First – CHIP Enrollees you can join to help your child manage their conditions or needs.

What can the Complex Care Management program do for me?

The Complex Care Management program can help you:

• Understand and manage your child’s chronic health conditions.
• Identify your child’s medical and non-medical needs.
• Understand your child’s medications.
• Set goals to improve your child’s health.
• Find the right doctors and specialists to manage your child’s care.
• Connect to available resources in your community.

If you are a part of but no longer want to be in the Complex Care Management program, call 1-844-377-2447 (TTY 711). Leaving the program will not change your child’s benefits. It will also not change the way Keystone First – CHIP or the Department of Human Services (DHS) treats you or your child.
