When you child becomes a Keystone First – CHIP Enrollee, you will get an ID card in the mail. Your child’s ID card will look like this:
The card includes your child’s personal Keystone First – CHIP ID number, as well as other important phone numbers and addresses for both you and your child’s health care providers. It is important to carry your child’s ID card with you at all times. You will need to show your child’s ID card to get the benefits and services your child needs that are covered by CHIP.
What if I lose my child’s Keystone First – CHIP ID card?
If you lose your child’s Keystone First – CHIP ID card, call Enrollee Services at 1-844-472-2447 (TTY 711) for a new one.
