Leaving the Hospital

Has your child just been discharged from the hospital? Do you need help with what to do next? Every day matters during recovery.

Keystone First – CHIP is here to help Enrollees stay healthy after their hospital stay. Getting well after your child’s hospital stay is important. But, sometimes, people end up back in the hospital soon after they leave. This is called a readmission. Readmissions can happen for a lot of reasons. By taking some simple steps, you may be able to help your child stay healthy and out of the hospital.

Before your child leaves the hospital

Getting ready before your child leaves the hospital can help your child get better faster. Just follow these easy tips:

  • Tell your child’s nurse where you plan to go after your child leaves the hospital.
  • Make sure you understand your child’s discharge instructions. Take them with you when your child leaves the hospital.
  • Know who to call if you have questions.
  • Get a list of the medicines your child will need to take at home.
  • Make sure you have a prescription for each medicine your child needs to take. You will need to give this to the pharmacist.

Is your child just out of the hospital?

Here's what you should do next:

  • Take your child to their doctor within 7 days. Write down your questions for the doctor before you go. Take your questions and discharge papers to your child’s appointment.
  • If your child has a nurse care manager, check in often. Your child’s nurse care manager will help you after your child leaves the hospital.
  • Follow the directions for giving your child medicine.
  • Ask your child’s pharmacist any questions you have about your child’s medicines.
  • Stick to your child’s recovery plan.
